This form is for referees to enter details of cautions (yellow cards) issued during junior matches. You are not required to enter details of temporary dismissals. If you have shown a red card, please complete a Referee Incident Report (separate form). Caution Codes Y1 – Delaying the restart of play Y2 – Dissent by word or action Y3 – Entering, re-entering or deliberately leaving the field of play without the referee’s permission Y4 – Failing to respect the required distance when play is restarted with a dropped ball, corner kick, free kick or throw-in Y5 – Persistent offences Y6 – Unsporting Behaviour
