Prescribed Form 19 – Claim for Obvious Error

  • Clubs should familiarise themselves with the relevant 2026 Disciplinary & Grievance Regulations prior to lodging a Claim for Obvious Error.
  • It is the responsibility of each Club to review the Match data of each of its Matches as recorded on the Competition Management System.
  • An obvious error occurs only when a Match Official’s decision is so clearly unreasonable that no reasonable Match Official could have made it, given all relevant information. Submissions must be supported by official video, photo, audio or written evidence.
  • Football West will only consider claims for Obvious Error that include all the necessary information. Any claim for Obvious Error that states “TBC” or “details to follow” or similar in relation to person responsible for the Offence will not be accepted.
  • This form must be lodged by either the President or Secretary of the Club. Submissions from other parties will not be accepted.
