Prescribed Form 19 – Claim for Obvious Error - Forms

Clubs should familiarise themselves with the relevant 2026 Disciplinary & Grievance Regulations prior to lodging a Claim for Obvious Error.
It is the responsibility of each Club to review the Match data of each of its Matches as recorded on the Competition Management System.
An obvious error occurs only when a Match Official’s decision is so clearly unreasonable that no reasonable Match Official could have made it, given all relevant information. Submissions must be supported by official video, photo, audio or written evidence.
Football West will only consider claims for Obvious Error that include all the necessary information. Any claim for Obvious Error that states “TBC” or “details to follow” or similar in relation to person responsible for the Offence will not be accepted.
This form must be lodged by either the President or Secretary of the Club. Submissions from other parties will not be accepted.

If Football West does not receive the submission of this form and relevant material within 48 hours of the Match being completed, the information in the Match Official Report will be deemed as fact and therefore no longer be appealable via a Claim for Obvious Error.

After considering the evidence, the Competition Management Committee (CMC) will determine whether the Claim for Obvious Error should be rejected or upheld.

Football West may conduct internal reviews of match footage and Match Official Reports to identify Obvious Errors, which shall be submitted to the CMC for determination.

The determination made by the CMC will be made in its absolute discretion and advised to the Club in writing. Any such determination is final and not subject to appeal (save that any determination in accordance with Regulation 5.4.4.1 shall be appealable as otherwise provided for in the 2026 Disciplinary & Grievance Regulations).
