Who should complete this form?

Any registered referee with the intention of officiating in the Sunday competitions (Amateurs, Women’s, Juniors, Metropolitan, Masters) and/or schools matches.

Referee Appointment
- 1 Appointment per Sunday
All referees will be eligible for appointment for 2 games each Sunday. If you wish to only be appointed to 1 game, please check the box above.
Junior Competitions*
Please nominate as either a Category 1 or Category 2 referee. Please read the appointments process CAREFULLY to understand the difference between Category 1 and Category 2 referees.
Junior Competitions Category 1*
Please advise the type of restriction (player, coach, parent etc.) including the age group, division and team you are associated with.
Conflict of Interest
Please advise if you have any conflicts of interest as per the Conflict of Interest policy including the age group(s), division(s) and team(s) you have a conflict of interest with.
Special Requests
Please advise if you have any special requests relating to referee appointments. Please note that all requests are considered however not guaranteed.
